摘要:背景:
吸烟对于类风湿关节炎(RA)发病的贡献,人们多认为是通过形成抗环瓜氨酸蛋白抗体(ACPA)发挥作用。然而,在RA患者体内通常同时存在ACPA、类风湿因子(RF)和抗氨基甲酰蛋白抗体(CarP)等多种自身抗体,吸烟是否只是特异性的与其中某些抗体产生相关而不与其他抗体产生相关尚不清楚。因此本研究旨在探讨吸烟是否仅与ACPA抗体产生相关,或也与其他的多种RA相关的自身抗体产生相关。
方法:
日本的一个以人群为基础的队列研究用于研究非RA人群中吸烟与RF和抗CCP2抗体产生的关系。随后对来自荷兰、英国和瑞典的三个早期关节炎队列进行分析。记录吸烟、RF、抗CCP2和抗CarP抗体相关的资料。计算自身抗体总评分,同时通过逻辑回归计算风险比和95%的可信区间。
结果:

BACKGROUND:Thecontributionofsmokingtorheumatoidarthritis(RA)ishypothesizedtobemediatedthroughformationofanti-citrullinatedproteinantibodies(ACPA).InRA,however,autoantibodiessuchasACPA,rheumatoidfactor(RF),andanti-carbamylatedproteinantibodies(anti-CarP)oftenoccurtogether,anditisthusunclearwhethersmokingisspecificallyassociatedwithsomeautoantibodiesratherthanothers.WethereforeinvestigatedwhethersmokingisonlyassociatedwithACPAorwiththepresenceofmultipleRA-relatedautoantibodies.
METHODS:Apopulation-basedJapanesecohort(n?=?)wasusedtoinvestigatetheassociationofsmokingwithRFandanti-cycliccitrullinatedpeptideantibodies(anti-CCP2)inindividualswithoutRA.Furthermore,RApatientsfulfillingthecriteriafromthreeearlyarthritiscohortsfromtheNetherlands(n?=?),theUnitedKingdom(n?=?),andSweden(n?=?)wereused.Dataonsmoking,RF,anti-CCP2,andanti-CarPwereavailable.Atotalscoreofautoantibodieswascalculated,andoddsratios(ORs)and95%confidenceintervals(95%CIs)werecalculatedbylogisticregression.
RESULTS:Inthepopulation-basednon-RAcohort,noassociationwasfoundbetweensmokingandoneautoantibody(RForanti-CCP2),butsmokingwasassociatedwithdouble-autoantibodypositivity(OR2.95,95%CI1.32-6.58).InRApatients,therewasnoassociationbetweensmokingandthepresenceofoneautoantibody(OR0.99,95%CI0.78-1.26),butsmokingwasassociatedwithdouble-autoantibodypositivity(OR1.32,95%CI1.04-1.68)andtriple-autoantibodypositivity(OR2.05,95%CI1.53-2.73).
CONCLUSIONS:SmokingisassociatedwiththeconcurrentpresenceofmultipleRA-associatedautoantibodiesratherthanjustACPA.ThisindicatesthatsmokingisariskfactorforbreakingtolerancetomultipleautoantigensinRA.
